We have a lady colleague who speaks so loudly in the office it's giving me and my colleagues a headache every morning.
To top it all off, she talks non-stop, even during meal times.

Perhaps, you can write a formal letter to her to inform about her voice. And you might want to include the signatures of your colleagues to explain the problem.

Consider a mediator if the problem gets out of control, or if the issue is too emotional to resolve in a mutual discussion. Your supervisor should be involved. You can consider using a neutral third party mediator within your own company (human resources if available) or hiring a professional counselor.

Find a suitable time i.e. lunch with her & imply that some walk-in customers or vendors could actually hear part of your conversation. This could mean some confidential discussions can be heard not only by those in the office, but sometimes outsiders as well.

I would indirectly broach the subject of her loudness through a lunch session with a group of our colleagues who share my opinion but do not wish to offend her. We would of course plan our topic of conversation to a fictional "friend" where the similar circumstances occur. We can then have an insight into her thoughts on this matter. If she is sensitive about the matter, we would have to drop the matter and buy ourselves ear plugs for a short term solution. For a long term solution, I would have to talk to her mentor. If she is open for discussion on this matter, then we just need to tell her the truth to see her reaction.
